Low-risk families often times fall through the cracks for assistance in our community. These programs are designed to offer low/moderate-low risk families a Public Health Nurse perspective on their child's needs, and to assist in their identifying and connecting to appropriate support, if needed.

Public Health Staff can accept referrals from Child Protective Services (CPS) for low-risk families with higher risk health needs. We offer home visits, assessment and case management services, focusing on developmental/growth parameters,  utilizing specific assessment tools which encourage quicker identification of a family's strengths and weaknesses, linking families with formal and informal services within their community.
