The Foster Care Passport Program's purpose is to compile and organize significant birth and health history on children placed in Foster Care for more than 90 days. Our goals include improving the documentation of existing health problems, tracking the numbers of children who receive needed medical treatment for physical and mental health problems, and the number of children in foster care who receive preventive health care.

We write a formal summary of all available medical, mental, behavioral and dental health history, and list current and former providers in order to assist foster parents, adoptive parents, and physicians to care for these children. This information is entered into the Children's Services computer database along with any family-of-origin significant medical history and educational information. This information follows the children wherever they reside so long as they are dependents of the state.

We also provide in-service training to medical providers, social workers, foster parents and other professionals regarding health-related issues impacting children in foster care.
