Island County Public Health established the Chronic Disease Prevention Program in January 2008. The program was established in response to state and local efforts that identified chronic disease as a priority health issue. The purpose of the program is to develop and implement effective interventions for communities, healthcare, schools, and worksites to address obesity and its consequent burden of chronic disease, including cardiovascular disease, diabetes, and asthma.
