Seniors/Vulnerable Adults Program

It is the purpose of this program to provide outreach, information and referral services to those seniors or vulnerable adults who have become isolated and or neglected and/or are experiencing depression and/or difficulties with medication and/or alcohol.

The case manager will make contact with the senior/vulnerable adult and focus on solutions through information, referral and case management that will benefit the client. Multiple visits may be necessary to ensure the client is following a case plan  and that quality of life is not diminishing.
