Jail Transition Services
The Jail Transition program was started in 2006 and assists inmates who have mental health, substance abuse and/or co-occurring disorders. This transition program includes many services designed to help inmates who are returning to the community. This will help engage them in a law-abiding lifestyle, live independently and assume family responsibilities through participation in treatment services, stable employment, housing and health care.
Goals of the program include decreasing recidivism, supporting inmates in becoming productive members of our community and getting individuals the support and treatment they need. Areas addressed are:
- Medical
- Psychosocial/Behavioral Health needs
- Legal Matters
- Insurance and benefit status and eligibility (including Medicaid, SSI, Food Benefits, etc.)
- Food
- Clothing
- Assistance with obtaining housing
- Vocational/Employment
